H-D schocks and rear rotor

I have a leaking shock and a grooved rear rotor on my 04 BD Chopper. Are H-D Softail shocks 89-99 or 00-04 interchangeable and is a 11.5 inch x 5mm rotor the standard rear rotor ? Can these be interchanged ?
