First gear

Fellow asked me the other day if my bike ever comes out of first gear when cold. I had never had that one come up as of yet. My first thoughts were shifting fork problem or the shifter pawl adjustment. Am I on the right track? He has a '06 ridgeback thnx fellows

The roller bearing that runs in the middle of the gear might be comming out. If it is, it needs to be replaced, and in stalled with green lock tight
