Clutch and Transmission Failures

Energy One
Just got my Dog back with the rebuilt transmission (Thanks Bigdog for covering it) and was wondering about the problems some of the dogs have been having with mainshafts and transmission problems. I've notice more than a few have started with the nut on the clutch basket coming loose. Seems after that the transmission failures start. Mine was the countershaft on the transmission had moved. Could this be cause by the nut on the clutch basket coming loose? I figure some of the experts on here could shed some light on the issue.

I know of one instance where the countershaft had shifted, never heard why it happened. I'd suspect that there was a bearing to shaft fitment issue but that's just a guess. the hub nut shouldn't have any relationship to the countershaft moving (at least as I've been able to figure out) but I'm by no means an expert.
