I almost lost mine a couple of times.
I made a new one this year that has aluminum angle on the outside of the bar (can't slide down) covered it with some fabris I bout at walmart. Will add pictures next time (too dark outside right now).

What do you mean, Its just wedged in there? I´ve two for my K9, (a 12 & a 18") .... both have bolts and they do not go anywhere in a hurry if you use blue loctite on the bolts!
Are you sure you didn't lose the bolts?

The holes should be at the very end, the bolts go in from the bottom.

Yep, my dumb ass lost two before I put 2 set screws in the last one.
Mine were getting pushed off the bar by the handle on my pack.

I run this one from JP cycles it doesn't fit perfectly but it works and I'm less worried about losing it. You could probably heat it up a bit to get it to fit better.

Here is a pic see one side fits the other is off a bit. It doesn't fall it's snug, it does sit up higher on the bar but no females have complained they actually prefer it up higher.
