Looks like you probably had a loose clutch hub nut at some point. It could cause your clutch to slip under a heavy load, but it would also start wearing the grooves on the plates as well so both together. It's not the worst I have seen but you've worn off the hardened coating so it will just continue to deteriorate.

Check the teeth on the inner hub that attaches to main shaft, see if those are worn.
